TODAY'S BRIEF:

  • Why you DON'T inherit baldness from your mother's father (the real genetics)

  • The testosterone derivative that's killing your hair follicles

  • What works: caffeine beats Rogaine, microneedling resurrects "dead zones," finasteride works but...

THE MAIN EVENT

The Real Reason Your Hair Is Falling Out

By age 50, half of all men and women notice significant hair loss. Not because it suddenly started at 50—it's been thinning since age 30.

The culprit: Dihydrotestosterone (DHT).

Your body converts testosterone into DHT using an enzyme called 5-alpha reductase. DHT binds to androgen receptors 5x more strongly than testosterone. That's great for strength, libido, and mental vigor. But DHT also miniaturizes hair follicles on your scalp, shortens growth phases, and eventually kills the stem cells that produce hair.

Why young men with high testosterone aren't bald: Because they haven't ramped up 5-alpha reductase production yet. It's not testosterone—it's the conversion to DHT.

The genetics myth: You DON'T inherit baldness from your mother's father. Pattern hair loss is polygenetic—multiple genes, not one clean line. Some people lose hair on the crown, some on the front, some everywhere. It depends on where YOUR androgen receptors are densest.

Why your beard gets thicker: DHT stimulates facial and back hair. Same hormone, opposite effects, different locations.

THE QUICK HIT: What Actually Works

Tier 1: Low Risk

Topical Caffeine: Blocks PDE (a growth brake), indirectly boosts IGF-1. Head-to-head studies show it's as effective as Minoxidil without blood pressure drops or prolactin spikes. Use 3x/week, leave on 3-5 minutes.

Ketoconazole Shampoo (2%): Antifungal that mildly reduces DHT. 80% response rate for maintaining hair. Use 2-4x/week, 3-5 minutes on scalp before rinsing. OTC versions are often only 1%—not strong enough.

Microneedling: Rolling tiny needles (1-2.5mm) over scalp triggers stem cells to reactivate. Microneedling + Minoxidil can regrow "dead zones" (completely bald patches) in 30-50 weeks. Once every 1-2 weeks. Will bleed—that's normal.

Tier 2: Moderate Risk

⚠️ Minoxidil (Rogaine): Dilates blood vessels, increases nutrient delivery. Slows hair loss, doesn't regrow much. Side effects: lowers blood pressure (dizziness, ankle swelling), increases prolactin (kills libido). Lifetime commitment—stop taking it, lose the hair.

⚠️ Saw Palmetto (300mg/day): Weak 5-alpha reductase inhibitor. Mild effect. Low side effects. Often combined with other treatments.

Tier 3: High Risk, High Reward

🚨 Finasteride: Blocks 5-alpha reductase, cuts DHT by 50% even at tiny doses (0.01mg).

Effectiveness:

  • 90% see reduced hair loss

  • 20% increase in hair count

  • 20-30% thicker hair

Dosing: 0.5-1mg/day oral, or 0.25% topical (1ml/week). Start LOW. Wait 25+ weeks before increasing.

Side effects:

  • Reduced libido, erectile dysfunction

  • Depression, loss of drive

  • Gynecomastia (male breast tissue)

Post-Finasteride Syndrome: Some men—especially 20s/30s—develop severe, lasting symptoms AFTER stopping: persistent sexual dysfunction, crushing depression, suicidal ideation. DHT likely plays ongoing roles in brain development well into your 30s. Blocking it may cause permanent changes.

The verdict: Finasteride works. But it's a lifetime commitment with serious risks. If you go this route, work with a doctor and monitor bloodwork.

🚨 Dutasteride: Blocks all three forms of 5-alpha reductase. Cuts DHT by 95%. Works 2-5x faster than finasteride. Also 2-5x more side effects. Only for the desperate.

YOUR WEEKLY WIN

Start with the safest options first.

If you're noticing thinning:

  1. Topical caffeine (3x/week)

  2. Ketoconazole shampoo 2% (2-4x/week)

  3. Microneedling (1x every 1-2 weeks)

If that's not enough, add Minoxidil or saw palmetto.

Only consider finasteride if:

  • You've tried everything else

  • You understand the risks

  • You're working with a doctor

  • You're committed for life

Critical: The combo of microneedling + finasteride is the most effective treatment. Dead zones can regrow in 30-50 weeks.

HELP US HELP YOU

That's your brief. Keep your boys cool.

- The Male Brief

P.S. If you're taking finasteride and experiencing low libido, brain fog, or depression—stop and talk to your doctor. Post-finasteride syndrome is real. Your hair isn't worth your mental health.
